Dear Annual Benefit Fund and/or Pension Fund Participants:

We are pleased to announce that the Trustees of both Plans have set up new services for your convenience. These services can be accessed through the www.ITPEbenefits.org website. Please click on the "Annual Benefit Login" or "Pension Login" links. This will allow you to access your individual account.

You can now make benefit requests, view your statement, download blank forms, change your profile, and access various other services. It's a fast and very handy tool accessible by all of our participants at any time of the day.

Once you complete your initial login as above you will be instructed to type in a new login and a new password to something personal you will remember, however it cannot be your social security number. You will then have to confirm your new password, so be sure to type the confirmation exactly the same. Then please be sure to provide your e-mail address in the required space.

You can save time by using this website whenever you need to. Give it a try and you will see how simple it is to access many of our services. ANNOUNCING NEW SERVICES
FOR PARTICIPANTS OF THE ITPE ANNUAL BENEFIT FUND

We will now have the capability to do Direct Deposit and ADP Total Pay Card. Direct Deposit allows us to automatically deposit your annual benefit money (vacation, sick, holiday pay, etc.) into your savings or checking account. Your money will be in your account on the day that it is issued. No more waiting for the post office to deliver it. There is no longer the possibility of your check being stolen by another or lost by the post office.

All you have to do is check off YES I WANT DIRECT DEPOSIT and mail or fax us this form and a check with the word VOID written across it. Please allow 3 pay periods for activation.

ADP Total Pay Card is another option you will have. ADP Total Pay Card is similar to direct deposit; however you don't need a bank account. Your money will be put into this card where you can access it at many in network ALLPOINT ATMs. You can make purchases at thousands of locations including stores like Wal-Mart. You will no longer have to pay check cashing fees or wait for the post office to deliver your check. More information on the pay card is enclosed, including fees for a lost card or out of network ATMs, etc. If you would like to enroll, please check off YES I WANT ADP TOTAL PAY CARD. We will then send you a card and an activation kit. You must activate your card with our office and the ADP Total Pay Card network once you receive it. Please allow 3 pay periods for activation.


Please note once your Direct Deposit or Pay Card is in effect you will only receive a check summary in the mail. If you do not choose any of the new options, you will continue to receive a regular check through the mail.

Important Information for ITPE Health & Welfare Fund Participants

Summary of Material Modifications

The Board of Trustees of the ITPE Health and Welfare Plan is pleased to announce the following benefit improvements will be effective January 1, 2009

Medical Plan - For Contribution Rates of $3.24 and above only.
(All Medical Benefits for Contribution Rates below $3.24 are unchanged.)

In-Network Only:
The Physician Services that follow are provided before you have to meet your annual deductible.


Physician Visits: Primary Care Physicians - Participant pays $20 per visit then the Plan pays 100%. Tests performed in the physician's office are included. The term Primary Care Physician means: General Practitioner, Internist, Family Practice Physician, and Pediatrician.

Physician Visits: Specialists - Participant pays $35 per visit then the Plan pays 100%. Tests performed in the physician's office are included.
In and Out of Network:
Deductible: $200 per Participant per Year, for All Services not performed in a Physician's office when In-Network, and All Services provided outside of the Blue Cross Preferred Provider Network.

Coinsurance and Out of Pocket Maximum Expense: Participants will pay 25% of allowable expenses In-Network, and 35% of expenses Out of Network up to an Annual Maximum of $2,000, In or Out of Network.

Annual Maximum Payments by the ITPE Health and Welfare Fund:
Class IV= $300,000
Class III= $250,000
Class II= $200,000
Class I= $140,000

Hearing Aid Benefit: Up to $2,000 over a 24 month period, paid like any other medical benefit after your deductible and coinsurance.

The following Medical Plan Improvement applies to All Contribution Rates

• Mental Health Benefit: Will also include coverage for Autism and Attention Deficit Disorder


Other Benefits

Prescription Drug Benefit - For all Contribution Rates except $1.63

• The Current Benefit, reimbursed at 75% by the Fund Administrator will have the Annual Family Maximum Benefit will be increased to the following amounts:
Class III and IV $10,000
Class I and II $7,000

Birth Control Benefit - For all Contribution Rates
• The Current Coverage, reimbursed at 100% by the Fund Administrator will be increased to $300 per year for female employees and female spouses of employees.

Dental Benefit - For all Contribution Rates
•The Current Coverage, reimbursed at 75% by the Fund Administrator will have the Annual Maximum Benefit increased to the following amounts.
Employee $2,000
Dependent $1,000

Vision Benefit - For all Contribution Rates
•The current allowance for an exam will be increased to $60 per Participant.


Weekly Accident and Sickness Benefit - For all Contribution Rates
Class IV: $200
Class III: $150
Class II: $90
Class I: $70

*** Note: You must fill out a New Enrollment Card When You Change Your Address, Add or Drop a Dependent. You must include Birth Certificates, Adoption Papers, Marriage License or any required Court Documents. Make sure Your Union Representative has a Copy of Your Enrollment Card.
